WHAT:                      Three Vehicle Fatal Traffic Crash

 

WHERE:                   Illinois Route 45/42 at 7000N Road, Kankakee County

 

WHEN:                      June 6, 2021 at approximately 4:09 p.m.

 

VEHICLE:                Unit 1 – 2014 Gray Toyota Camry

                                    Unit 2 – 2019 White Acura ADX

                                    Unit 3 – 2014 Silver Nissan Maxima

 

DRIVER:                   Unit 1 – David L. Raymond, 57-year-old male of Kankakee, IL – Deceased

                                    Unit 2 – Cynthia S. Alvarado, 25-year-old female of Manhattan, IL

                                    Unit 3 – Ethan G. Hack, 20-year-old male of Kankakee, IL – Transported to a local hospital with injuries

 

PASSENGER:           Unit 3 – Justin K. Hack, 18-year-old male of Kankakee, IL

                                    Unit 3 – Juvenile, 15-year-old male of Kankakee, IL

 

PRELIMINARY:     A preliminary investigation indicates the following occurred: Unit 1 was traveling southbound on Illinois Route 45/52 and for unknown reasons entered the northbound lanes striking Unit 2, which was traveling northbound on Illinois Route 45/52, head on.  Unit 3 was traveling southbound behind Unit 1 and struck Unit 1 following the impact of Unit 1 and Unit 2's crash. The driver and sole occupant of Unit 1 was pronounced deceased at the scene.  The driver of Unit 2 did not sustain any injuries.  The driver of Unit 3 was transported to a local hospital with injuries.
