District 21
WHAT: Double Fatal, Pickup vs. Semi
WHERE: Route 45/52 @ 8500S (Kankakee County)
WHEN: Thursday, October 17, 2019, 6:01 p.m.
VEHICLE: Unit One: 2002 Dodge Dakota
Unit Two: 2013 Volvo Truck Tractor-Semi Trailer
DRIVER: Unit One: Clifford J. Moczynski, 34, Kankakee (Deceased)
Unit Two: Timothy L. Glaze, 57, Kankakee
PASSENGER: Unit One: Cynthia Slavin, 60, Kankakee (Deceased)
PRELIMINARY: Preliminary investigation has indicated the Dodge was traveling
westbound on 8000S. The Semi was traveling northbound on Route
45/52. The Dodge failed to yield at the stop intersection and collided with
Semi. The driver and passenger of the Dodge were pronounced
deceased on scene by the Kankakee County Coroner. The driver of the
semi was transported to a local hospital with injuries.
